Integrated Step-Down Process Preparing Young Adults for Aftercare in Orem, UT

Our transitional living program in Utah serves as the connective tissue between more structured treatment options and independent living, teaching the life skills an adolescent needs to thrive on their own.

Supporting Young Adults During Their Transition to Independence

That feeling of “now what?” after a highly structured program can be terrifying for young adults, even if they feel ready to make the change. After weeks or even months of having treatment programs that provide them stability, our transitional living program will help them stay grounded and focused on their path toward independence.

Our transitional living program incorporates a step-down process for young adults that is highly individualized and tailored to each client’s needs. Rather than relying on a single standardized model, we match each young adult’s goals and capabilities to support lasting progress.

Why Support Matters for Long-Term Success

Moving from an intensive program into the world is a major transition that requires support from people invested in your long-term success.

Maintaining Progress During Transitional Living

Without continued structure during transition, the new habits a young adult built in treatment can quickly fade. On-site peer support and involvement from family members while residents are enrolled help reinforce the tools they developed in treatment so they can keep their hard-won progress.

Strengthening Mental Health and Well-Being

Consistent professional support helps emerging adults manage their stress, identify stressors, and handle the big emotions that come with figuring things out during early adulthood.

Building Confidence in Independence

Stepping out on their own takes practice, especially after receiving such a high level of care and support. Our approach to support is all about practice. Students will live their lives in a way that keeps the stakes entirely manageable, so they don’t feel overwhelmed by responsibility or decision-making.

Staying Connected to a Support System

A strong community is a strong foundation. Having a support system is excellent for maintaining relationships with mentors and peers who give young people the courage to succeed. Beyond clinical oversight, our program provides the emotional anchor of consistent, high-value relationships built on mentorship and accountability.

How Our Program Prepares Students for Independent Living

Our integrated step-down process during their time with us provides tailored support based on each young adult’s needs. It’s important that they can feel this new level of autonomy without ever feeling micromanaged or overwhelmed.

Check-Ins and Accountability

Regular check-ins with a treatment team become their anchor. This helps them stay focused on their goals.

Mentorship and Guidance

Dedicated mentors provide hands-on help with this transition period. They know just how hard it is to make so many new decisions, but they will help you navigate these complexities with a more compassionate and even collaborative level of care.

Skill-Building

Therapy is an important part of building a healthy foundation, but our program focuses on where students can put what they’ve learned into practice. They keep honing their life skills, like time management and effective communication.

Flexible Support Based on Individual Needs

Support needs to be adaptive. That’s why our step-down process is intentionally designed to shift based on what the adolescent needs, whether it’s more focus on executive function or help managing emotions.

Supporting the Path to Independent Living

Independence is something that many people gain, but it’s also something you have to learn. We’re here to help students practice it until it becomes second nature.

From Structure to Self-Sufficiency

The goal of STRiV is to help young adults shift from guided support to managing their own lives with confidence. Completing this program means they’ve not only solidified their existing structure but are also capable of creating their own when life throws new challenges their way.

Navigating School, Work, and Daily Life

Whether it’s enrolling in college classes, job searching, or just building the beginnings of a healthy daily routine, we’re right there with them. This is where our support goes beyond the theoretical and into the messy realities of life.

Real-World Problem Solving

Life is a series of unpredictable problems that we learn how to solve ourselves. Our team doesn’t solve problems for an adolescent in our program. We teach them how to come to their own conclusions without overextending. Friction is a fact of life, so we want them to know they can rely on themselves when it shows up, rather than looking for a way out.

Maintaining Healthy Habits

Sometimes, mental and physical health are simpler than we realize. We show young adults just how much sleep, nutrition, movement, and sticking to a routine matter and play a major role in their long-term stability.

Mental Health and Therapeutic Support

We prioritize mental health and wellness as part of the umbrella of physical health. Every student in the program should feel like they can understand their thoughts so they can take better actions.

Therapy and Support Groups

As they face new situations that may challenge their progress, young adults can continue individual therapy, group therapy, or support groups. This gives them a place to process their experiences while holding them accountable.

Evidence-Based Approaches

Proven methods, such as cognitive-behavioral therapy (CBT), remain a core part of their support. It’s one of the most concrete tools to help manage anxiety, intrusive thoughts, and many other mental health issues that everyone needs help with.

Collaboration With Providers

Continuity of care is a must for positive outcomes. STRiV will gladly coordinate with outside healthcare providers, therapists, or clinicians so that nothing falls through the cracks.

Medication Management Support

We coordinate with prescribing providers as needed to support medication management and help students maintain the stability they’ve worked hard to achieve.

Following Different Levels of Support

Stepping down from a more structured environment is only successful if there’s solid ground beneath them. Care following treatment can be an incredibly valuable next step for young adults transitioning from:
  • Residential treatment or residential care
  • Intensive outpatient program (IOP)
  • Other structured treatment programs
Rather than stepping down into less support, our process provides the right kind of support for continued growth. STRiV Forward helps them translate the clinical gains they’ve made into real-world successes — things they can feel they’ve achieved themselves.

Support for Families and Loved Ones

When a young adult transitions out of an intensive care program, they aren’t the only ones adjusting to a new normal. The entire family can feel the change.

We actively involve a student’s parents and loved ones by providing resources, guidance, family therapy (when needed), and referrals to help them acclimate to a recovering adolescent’s newfound independence. The hope is that this will be a combined effort to ensure the entire system grows toward a healthier future, both for the student and the family unit.

Who Benefits from Our Program?

Our step-down process is designed for any young adult or adolescent who has put in the hard work to stabilize their mental health but still needs additional support to ensure their work sticks with them. Many students navigating neurodivergence, managing a mood disorder, or managing mental health issues just need a way to find their footing. We offer them a welcoming, non-judgmental place to prioritize their mental health and find long-term success.

A Continued Path Toward Independence

Healing is complicated. And while time may heal all wounds, everyone’s timeline is different, and their pain or struggles can make things even more complicated. By choosing a program that emphasizes empowerment and agency, our students complete their program with the confidence to let go of the past and own their futures.

FAQs

Transitional living for young adults is a structured, supportive program that helps them move from more intensive care toward independent adulthood. It combines housing, life-skills training, clinical support, and mentoring tailored to each individual’s needs. The goal is to build practical independence, emotional stability, and community connections for a successful long-term transition.

STRiV’s step-down process is built into the program to help residents move from more intensive clinical care into greater independence. This support focuses on practical life skills training, mentorship, and appropriate clinical coordination to help students apply clinical gains in everyday settings.

No. These step-down elements are integrated into STRiV’s program rather than offered as a separate, ongoing aftercare program. While we encourage ongoing support and aftercare planning, we do not offer a standalone aftercare service.

Services include regular check-ins, executive-function coaching, individual and group therapy, coordination with prescribing providers when appropriate, and one-to-one mentorship — each tailored to the resident’s needs. Each support component is matched to the resident’s clinical and functional goals and tracked to monitor progress.

Yes. Residents have access to individual and group therapy through the program, with coordination with external providers as needed to maintain continuity of care. All therapeutic services are delivered by credentialed clinicians and integrated into the resident’s care plan.

Yes. By reinforcing routines, coping strategies, and problem-solving skills while increasing responsibility, the step-down elements help reduce the risk of regression and support a smoother transition to independent living. Outcomes are regularly reviewed so interventions can be adjusted promptly if risks or setbacks emerge.
